These forms must be used for each and every ejection or disruption that occurs in games that you work. It doesn’t matter if it is a league game, playoff, tournament, provincial or exhibition contest.
The form should be filled out as soon after the incident as possible so that the information is fresh in your mind.
if you eject try to write down the important information as soon as possible using your lineup card or the actual score sheet.
The information will go to the local executive who will then take action.
